USERESTI LA PREP? RISULTATI DA UN SONDAGGIO NAZIONALE TRA GLI MSM IN ITALIA

Obiettivo: Valutare l’attuale conoscenza e le intenzioni di utilizzo futuro della profilassi pre-esposizione (PrEP) tra i maschi che fanno sesso con maschi (MSM) in Italia e indagare come l’uso della PrEP potrebbe influenzare il comportamento sessuale degli MSM.
Metodi: Sondaggio via internet auto-somministrato da 503 MSM tra febbraio e marzo 2013
Risultati: La maggior parte dei partecipanti erano sessualmente attivi ma più della metà di loro (53,48%) non aveva mai sentito parlare della PrEP. PrEP è accettabile per il 64,41% dei partecipanti; il 48,51% la userebbe solo sotto controllo medico e il 15,90% persino senza; PrEP è ancora più accettabile tra coloro che non ne avevano mai sentito parlare (67,66% contro 60,00% tra coloro che conoscevano la PrEP, p=0.019).
Tra coloro che non userebbero la PrEP e non sanno se la userebbero (rispettivamente il 17,69% e il 17,89%), più della metà è preoccupato dalla sua affidabilità (29,05% non pensa sia affidabile per niente e il 26,26% non sa quanto sia affidabile), con coloro che erano già informati sulla PrEP più propensi a pensare che non sia affidabile per niente (35,56% contro 21,84%, p=0.007). Il 45,25% di coloro che non userebbero la PrEP pensa che essa dovrebbe essere disponibile per altre persone; il 33,52% pensa che non dovrebbe essere disponibile perché porterebbe a un minore utilizzo del preservativo.
Tra coloro che userebbero la PrEP, il 53,70% preferirebbe una strategia intermittente; il 59,57% farebbe sesso con la stessa frequenza con cui lo fa ora; il 29,01% cambierebbe l’utilizzo del preservativo mentre gli altri continuerebbero a usare il preservativo “sempre”, “quasi sempre” o “mai o quasi mai” (37,35%, 17,90% e 11,42% rispettivamente).
Conclusioni: Servono maggiori informazioni e metodi di impiego più semplici per la profilassi pre-esposizione per rispondere ai bisogni degli MSM. Per molti partecipanti la PrEP sembra essere un metodo di prevenzione accettabile e non userebbero i preservativi meno frequentemente.

Would you use PrEP? Results from a national survey among MSM in Italy

Objective To assess current knowledge and intended future use of pre-exposure prophylaxis (PrEP) among men who have sex with men (MSM) in Italy and investigating how PrEP use could affect the sexual behaviour of MSM.
Methods Self-administered web-based survey of 503 MSM on February and March 2013.
Results Most of responders were sexually active but more than half of them (53,48%) never heard about PrEP. PrEP is acceptable for 64,41% of all the responders; 48,51% would use it only with medical control and 15,90% even without; PrEP is even more interesting for those who did not hear about it (67,66% vs 60,00% among those who knew about PrEP, p=0.019).

Among those who would not use PrEP or do not know whether they would use it (respectively 17,69% and 17,89%), more than one half is concerned about its reliability (29,05% do not think it is reliable at all and 26,26% do not know how reliable it is), with people already informed about PrEP more likely to think it is not reliable at all (35,56% vs 21,84%, p=0.007). 45,25% of those who would not take PrEP thinks it should be available for other people; 33,52% think it should not be available because it would lead to lower condom use.
Among those who would use PrEP, 53,70% would prefer an intermittent strategy; 59,57% would have sex with the same frequency they have now; 29,01% would change his condom use while the others would continue to “always”, “almost always” or “never or almost never” use condoms (37,35%, 17,90% and 11,42% respectively).
Conclusions More information is needed and more effective and easy-to-use methods for pre-exposure prophylaxis are needed to meet MSM needs. For many responders PrEP seems to be an acceptable prevention method and they would not use less frequently condoms.
